[摘 要] 目的 探讨BiPAP在合并Ⅱ型呼吸衰竭患者的COPD患者中的应用的效果.方法 便利选取该院2015月6月—2016年12月期间呼吸内科收治的60例慢性阻塞性肺疾病合并II型呼吸衰竭患者为例,随机将其分为观察组和对照组,每组30例.对照组患者仅给予常规方法治疗,观察组患者在对照组基础上联合BiPAP连续治疗.直至患者出院,统计两组患者的平均住院时间、气管插管率,同时检测患者治疗前后肺功能及血气分析各项指标变化情况.结果 观察组患者住院时间平均（10.4±2.1）d,氣管插管率6.67%均显著低于对照组（15.2±2.7）d、16.67%,两组比较差异有统计学意义（P<0.05）.两组患者治疗后血气分析、肺功能各项指标较治疗前均有明显改善,且观察组各项指标均显著优于对照组,差异有统计学意义（P<0.05）.结论 BiPAP对于治疗合并Ⅱ型呼吸衰竭的COPD效果较好,不但能显著改善患者血气指标和肺功能,更能缩短住院时间,减少插管率,有助于预后,值得推广.

[Abstract] Objective To study the application effect of BiPAP in patients with COPD and Type II respiratory failure. Methods Convenient selection 60 cases of patients with COPD and type II respiratory failure admitted and treated in our hospital from June 2015 to December 2016 were selected and randomly divided into two groups with 30 cases in each, the control group adopted the routine method, while the observation group adopted the BiPAP continuous treatment on the basis of the control group, and the erage length of stay and tracheal intubation rate of the two groups were counted until the patients were discharged, at the same time, the changes of various indexes of lung function and blood gas analysis of patients were tested. Results The erage length of stay and tracheal intubation rate in the observation group were obviously lower than those in the control group[（10.4±2.1）d, 6.67% vs （15.2±2.7）d, 16.67%], and the differences between the two groups were statistically significant（P<0.05）, and various indexes of blood gas analysis and lung function after treatment were obviously improved compared with those before treatment, and various indexes in the observation group were obviously better than those in the control group, and the differences were statistically significant（P<0.05）, and various indexes in the observation group were obviously better than those in the control group, and the differences were statistically significant（P<0.05）. Conclusion The treatment effect of BiPAP in patients with COPD and type II respiratory failure is better, which can not only obviously improve the blood gas index and lung function of patients, but also shorten the length of stay, reduce the intubation rate and improve the prognosis, which is worth promotion.
